Please read the statements and points below.  By submitting this Application for Employment form (Harvest Season), it is deemed that you agree and accept these terms and conditions.

  1. By submitting this form, I certify that when I show up for my shift on any given day, I am fit to carry out my work duties.
  2. Fit for work means:
    1. I am without ANY injury
    2. I am not feeling sick (medical or other)
    3. I am prepared to work for the normal 8.00-hour day and am prepared physically and mentally.
    4. I fully understand the physical nature of my duties and fully understand that should I not be fit for work; I would be unable to carry out my duties.
  3. Unfit for work means:
    1. I am with injury
    2. I am feeling sick (medically, mentally, depression, personal and other causes)
  4. I have a full understanding of what is required at work and I am fit to carry out my duties.
  5. I will NOT turn up for work on any given day if I am feeling that I am NOT fit for work.
  6. I will NOT continue working on a day should I become unfit for work during the course of my shift and I will stop working immediately and inform my supervisor of the reason I am identifying that I am unfit to continue the rest of my shift.
  7. I understand that Garshey’s may terminate my employment on the grounds that I am unable to continue working on a day to day basis due to my being unfit for my duties.
  8. I agree to these terms and conditions in this Application for Employment form without coercion or under duress.
  9. My supervisor has explained to me that it is unlawful to put my name to and sign on any document that I haven’t read, understood or not truthful.
